Can anyone direct me to a website that would have an example of a Distributor Application for an MLM Company?

MLM – MultiLevel Marketing also know as NM – Network Marketing. Just want to look at what distributors usually have to sign in those companies when they pay their application fee to get the starter kit. Any general mlm company would do, doesn’t have to be anything specific. Examples of MLM Companies – Amway/Quixtar, Herbalife, USANA, Longevity, Prepaid Legal Services, etc…
